A passenger was thrown to the ground after a train travelled at almost twice the speed limit near an Elizabeth line station.
The Rail Accident Investigation Branch (RAIB) is investigating the incident, which took place during rush hour on September 24 just east of Manor Park station.
The Elizabeth line train passed over a set of points travelling at 45mph – 20mph over the maximum speed limit.
This caused the train to jolt sideways, throwing at least one passenger to the ground and causing others to lose their footing.
There were no reported injuries and the train did not derail during the incident, a preliminary investigation has found.
The RAIB also found that no damage was caused to the railway or to the train involved.
Rail investigators will now publish a safety digest setting out what can be learned from the incident.
Manor Park station is only served by Elizabeth line trains, but lies on the Great Eastern Main Line, where Greater Anglia and c2c services also run.
